Poppy’s Home Care offers support through the Home Care Package Program (HCP), which delivers tailored home care packages designed to assist individuals with their care needs to remain comfortable and independent in their own homes. Covering all suburbs between Kwinana and Dawesville and the Perth Metro areas.
Care And Support We Offer
The services we provide under the HCP program encompass a wide range of support, including:
Personal Services
Social Support
Helping individuals participate in community life and to foster social connections through group-based activities.
Nutrition, Hydration, Meal
Preparation, and Diet
Support with meal preparation, including specialised diets for health, religious, cultural, or personal reasons, as well as assistance with using eating utensils and feeding.
Continence Management
Assistance with the use of continence aids and appliances, including disposable pads, absorbent aids, commode chairs, bedpans, urinals, catheter and urinary drainage appliances.
Mobility and Dexterity
Provision of mobility aids by working in partnership with Physio and Occupational Therapist for equipment such as walkers, frames, mechanical lifting devices, bed rails, slide sheets, pressure-relieving mattresses, along with support in using these aids.
Nursing, Allied Health,
and Clinical Services
Access to a variety of health services including speech therapy, dietary support, podiatry, occupational or physiotherapy services, and other specialist health and well-being services as required.
Transport and
Personal Assistance
Help with shopping, visiting healthcare providers, and attending social activities.
Management of
Skin Integrity
Assistance with bandages, dressings, and the application of skin emollients to maintain and improve skin health.
Home Care Package Levels
At Poppy’s Home Care, we understand that every individual has unique care needs. To support older people in living safely and independently at home, the Australian Government offers Home Care Packages through the My Aged Care system.
These packages provide flexible, government-subsidised support tailored to individual circumstances. Eligibility and funding levels are assessed and determined by My Aged Care following an independent assessment.

Level 1: Basic Care Needs
This level provides support for individuals who require minimal assistance with daily activities.
Services may include:

- Coordination of specialised services, transport and shopping

If you’re not eligible for a government-funded Home Care Package or are waiting for approval, you can still access our professional and compassionate services as a private client.
Becoming a private client allows you to receive the same high-quality, personalised care whilst waiting for government funding.
