What services does Poppy's Home Care offer?

Poppy’s provides personalised Support at Home services based on your assessed needs, approved support plan and available funding.

Services may include personal care, nursing and clinical care, allied health, domestic assistance, meal preparation, social support, transport, respite care, assistive technology and home modifications. We work with you to arrange the right mix of services to support your safety, independence, wellbeing and connection to your community.

How do I know if I I’m eligible for Support at Home?

Eligibility for Support at Home is determined through an aged care assessment arranged by My Aged Care. The assessor will discuss your needs, goals and circumstances and determine the services and funding you may be approved to receive.

You can request an assessment by calling My Aged Care on 1800 200 422 or applying online. If you’re unsure where to begin, Poppy’s can explain the process and help you understand your assessment outcome.

How much does Support at Home cost?

Poppy’s Home Care is committed to clear, transparent pricing with no hidden fees. We do not charge entry or exit fees, and our standard prices are published on our website and through My Aged Care.

The amount you pay will depend on the services you receive and any participant contribution determined by Services Australia. We will explain all expected costs and include them in your Service Agreement and individual budget before services begin. You can download our pricing schedule here Pricing Schedule

Can I choose my care worker?

We’ll ask about your preferences and work to match you with Home Care Team Members who suit your needs, personality and routine. You can request a particular team member, and we will do our best to accommodate your choice, subject to their skills and availability.

Consistency and trusting relationships matter to us. If a match doesn’t feel right, please tell us so we can work with you to find a better fit.

How do you ensure the quality of care provided?

Poppy’s works in line with the strengthened Aged Care Quality Standards and has systems to support safe, respectful and reliable care.

Our Home Care Team Members are carefully screened, appropriately trained and supported through ongoing supervision and competency checks. As a nurse-led provider, we also bring clinical oversight to care planning, reviews and changing health needs. Feedback, incidents and service outcomes are regularly reviewed to help us continually improve.

What happens if my care needs change over time?

Your care plan is reviewed with you and can be updated as your needs, goals or circumstances change. We can adjust your services within your approved support plan and available budget.

If your needs change significantly, Poppy’s can help request a Support Plan Review or reassessment through My Aged Care to determine whether additional services, a different classification or other supports may be appropriate.

How does my feedback help improve care?

We welcome all feedback—positive or negative—and use it to improve our care and services. Feedback is recorded, reviewed and acted on, with any patterns or opportunities for improvement shared with the appropriate team members.

You can raise feedback or concerns with us at any time, and doing so will not affect the care and support you receive. We’ll listen, keep you informed and work with you toward a fair and respectful resolution.

How do I get started with Poppy's Home Care?

Getting started begins with a friendly, no-obligation conversation. Contact us by phone, email or through our website and we’ll take the time to understand what matters to you, explain how Poppy’s can help and answer your questions.

If you choose Poppy’s, we’ll review your Support at Home approval with you, develop your care plan and individual budget, complete your Service Agreement and arrange your services. If you’re changing providers, we’ll also guide you through the transition.

Understanding Support at Home

Support at Home helps older people live independently at home for longer by providing access to approved care and support services, assistive technology and home modifications.

Following an aged care assessment, you may be approved for one of eight ongoing funding classifications or short-term support based on your assessed needs. Poppy’s will help you understand your approval, plan your services and make the most of the support available to you.

Support at Home has eight ongoing funding classifications. Your classification and quarterly budget are determined through an aged care assessment based on your individual needs.

Separate funding may be available for approved equipment and home modifications that support your assessed needs and help you remain safe and independent at home.

Support at Home services are grouped into three contribution categories. The amount you contribute depends on the service category and your financial circumstances.

If you were receiving, approved for, or waiting for a Home Care Package on or before 12 September 2024, the grandfathered participant contribution arrangements may apply to you.
